A type of health insurance plan that usually limits coverage to care from providers who work for or contract with the HMO. HMOs require you to choose a primary care physician (PCP) and you’ll need to get a referral from your PCP to access care from a specialist. If you get care from an out-of-network provider (one that does not work for or contract with the HMO), the HMO plan generally won’t cover the care, except in an emergency. An HMO may also require you to live or work in its service area to be eligible for coverage. HMOs often provide integrated care and focus on prevention and wellness.
